AUTHORIZATION
I
authorize all previous employers, law
enforcement agencies, and individuals with
personal knowledge to disclose to Mackinac
Employee Services, L.L.C. (MES), or to its
agent, any and all information in their
possession about my employment history
(including disciplinary and other matters),
personal background and criminal record. I
hereby waive written or other notice to me
of the release of any information to MES. I
further authorize the educational
institutions listed in my application for
employment to disclose to MES any and all
information in their possession regarding my
attendance and performance at such
institution, including but not limited to
disclosure of any diploma or degree of
certification awarded, disclosure of
academic information and transcripts, and
disclosure of any disciplinary record. I
hereby waive written or other notice from
educational institutions of their release of
information to MES.
I
understand and agree that an investigation
may be made either by MES, through an
agency, or through a group of cooperation
employers to evaluate me for possible
employment. Information for investigative
reports may be obtained through personal
interviews with family, neighbors, business
associates, friends, or others who might be
acquainted with me. Information sought may
relate to my character, general reputation,
or personal characteristics.
For
the purpose of this Authorization and
Wavier, a photocopy of my signature shall
have the same force and effect as my
original signature.
TERMS
OF EMPLOYMENT
I represent that my statements are true and complete. I have
withheld nothing which, if disclosed, would affect this application unfavorably. I understand that if Mackinac Employee Services, L.L.C. (MES), at any time should determine that any of the requested
information was withheld by me or any of the statements furnished above was false or misleading, I may be discharged.
In the event of my employment, I will comply with all rules, regulations, policies, and communications directed to employees.
I understand that I will be free to resign my employment at any time with or without cause, and with or without giving reason, prior notice, or warning to MES; I agree that MES also may terminate my employment at any time, with or without cause and with or without prior review, notice, warning, or reason.
I acknowledge that no person acting for MES other than its President and no person at Grand Hotel other than its President can make any representation, promises, or guarantees as to my salary or earnings potential, and any such agreement must be in writing.
I agree to submit to physical examination permitted by law during my employment, at the request and expense of MES, and I agree to disclose completely all information lawfully requested at such examinations about my physical and mental condition and medical history. I
also agree that after an offer of employment and during my employment, at the request and expense of MES, I will cooperate in such lawful medical tests (including blood, urine, or other testing) as MES requests to check
for drugs or alcohol in my system.
I agree that the contents of any room, office, work space, desk, locker, lodging vehicle, or other property of Grand Hotel I may be using, and of my person and any of my own
property I bring onto Grand Hotel’s premises (including without limitation packages, suitcases, trunks, backpacks, briefcases, and purses) may be inspected by Grand Hotel at any time, and I waive and promise not to make any claims against Grand Hotel ( or its employees or agents) relating to such inspection. I understand that if I refuse to permit inspection, I will be terminated by MES immediately.
I agree that except as directed otherwise in writing by MES or Grand Hotel, I will not disclose to anyone or use for my own purposes any MES’s or Grand Hotel’s confidential or propriety information, either during or after my employment. I understand and agree that guest names and information, information about or arrangements with guest and convention referral sources, financial data, and computer information and
processing are confidential and proprietary information and I will not make written or other copies of or notes regarding these matters except as necessary to perform my job. I agree that when my employment ends, or at any time at MES’s or Grand Hotel’s request, I will deliver to MES or Grand Hotel all material of any kind that I have relating to Grand Hotel’s business including any such copies or notes. I agree that I will not make, and will not cause to be made, any visual or voice recording of any person, including guests and other employees,
without the express authorization of the President of Grand Hotel.
I agree that if any of the above commitments by me is ever found to be legally unenforceable as written, the particular agreement concerned shall be limited to allow its enforcement as far as legally possible.
I understand that if employed I will be required to
sign an Employment Agreement covering specific terms of my employment, which agreement shall be in addition to the Terms of Employment contained herein, and both of
which together shall constitute my entire
agreement for employment with MES.
I agree to the above terms of employment if I am employed by MES. I understand and agree that these provisions of my employment can be revised only by a written contract signed by the President of MES, and that no other person
to MES has any authority to offer employment other than on an at-will basis as described above. I understand and agree that, except as provided above, all benefits, programs, rules, and policies of MES are subject to exceptions or change at any time as decided by it in its sole discretion.
